Of course, health care is never that straightforward. Human beings also have psychological, social and spiritual belief and value systems alongside personal experience that blend to form each unique individual.

Like the USA the impetus for the development of specialist/ advanced roles in nursing was due to an anticipated shortage of junior medical staff in the UK resulting from the reduction in junior doctors’ hours. Despite a proliferation of specialist and advanced roles in the UK over the last two decades, the regulation of higher levels of practice has been complex and ambiguous. The UKCC opened a register for specialist practitioners, stipulating that entry to the register would require completion of validated educational programmes to at least first degree level or master’s level as well as a minimum of three years post-registration experience within the specific field of practice.

Therefore, it is imperative that a comprehensive assessment of patients and development of their rehabilitation goals is undertaken as soon as they are medically stabilised. The specialist nurse must support staff on acute units to actively participate in the assessment and goal planning process with the patient and their family. In addition, there needs to be educational support for acute staff to enable them to gain competence and confidence in specific rehabilitation skills.
